Paper Form Conversion

1. Paper form - this can be sent to us via USPS. You may also scan form and save as JPEG or PDF - upload to our server or e-mail it as an attachment.

2. Company logo graphic file, if required. This can be in PSD, GIF, BMP, or JPG format. May also be sent with paper form for scanning.

3. Notations of changes to be made on your original form.

System Requirements: PC and MAC users will require Adobe Reader 9.0 or Higher to be installed in order to save entered form data once converted. MAC Preview does not allow saving data.

Apple iPad users require current PDF Expert to be installed to save entered form data. Android Tablets require current RepliGo Reader app to be installed.

Acrobat Reader Extension Activation: Orders for PDF forms are e-mailed once completed - normally within 48 hours.

Order completion: You will be sent to our confirmation page and can upload your PDF or Word Doc form to our server for electronic form conversion or Adobe Reader Activation at that time - we are automatically notified. For paper form conversions, see the side bar for information.

Your CD order for form conversion, if required, will be shipped within 1 week via USPS - confirmation tracking number will be e-mailed at that time.

You may also upload your files directly to our server, utilizing the file upload boxes located on the order confirmation page.

A proof sample will be be sent for your approval prior to creating your final version and transferring files onto a CD.
