Information Required

Paper Form Conversion

1. Paper form - this can be sent to us via USPS. You may also scan form and save as JPEG or PDF - upload to our server or e-mail it.

2. Company logo graphic file, if required. This can be in PSD, GIF, BMP, or JPG format. May also be sent with paper form for scanning.

3. Notations of changes to be made on your original form.

All payments are processed through the CC Now® or 2Checkout.com secure servers which are designed for safe electronic transactions.

Your CD order for paper form conversion will be shipped within 1 week via USPS - confirmation tracking number will be e-mailed at that time.

Acrobat Reader Extension Activation orders are e-mailed once completed - normally within 48 hours. Upon order completion, you will be sent to our confirmation page and you may upload your PDF form to our server for activation at that time.

Upon check-out, you will be sent an e-mail directing you to our confirmation page. Full instructions are included for sending your paper form, either via USPS or scanning and e-mailing it to us.

You may also upload your files directly to our server, utilizing the file upload boxes located on the confirmation page.

Once reviewed, we will contact and verify the information to be inserted during conversion to electronic PDF form. A sample will be be sent for your approval prior to creating your installer package and transferring onto CD.
